South Bristol Youth works in partnership with the Future Quest team at UWE. We need to show what difference Future Quest has made to the young people involved and to do this we would like to collect data to allow us to follow your educational progress and to monitor and evaluate Future Quest activities. This will include the data collected on this form and the one completed by your parents or carers, details of your participation in our activities, your responses to our surveys and any other data you provide to Future Quest. The Future Quest team also collects your school email address from your school which they share with South Bristol Youth.

YOUR PHOTOGRAPHS AND SOCIAL MEDIA

Future Quest, South Bristol Youth and the people who deliver our activities like to take photographs and record videos to promote our work and your achievements. These images may be used for publicity on printed materials, websites and on social media including (but not limited to) Facebook, Instagram, Twitter and YouTube.

I give permission for Future Quest, South Bristol Youth and the people delivering activities to use my image as described above.

OUR AGREEMENT WITH YOU

South Bristol Youth staff would like all participants to benefit from the programme and to enjoy the new experiences. We wish to create a positive, safe and happy environment for all students and we will not tolerate student behaviour which prevents this. South Bristol Youth agree to provide you with high quality activities in a safe environment where everyone is respected. In signing this form, you agree to:

  • Fully engage and participate in each activity.
  • Show consideration and respect to all students, staff, ambassadors and the working environments.
  • Listen and follow instructions from all staff and ambassadors.
